HEALTH IMPROVEMENT PRACTITIONER (HIP)
This role will see you working at one of our Auckland based GP clinics providing mental and physical health and general well-being support. The Health Improvement Practitioner role is based on the Behavioural Health Consultancy (BHC) model, which has been tested and adapted for the New Zealand setting. This a generalist role, working alongside the General Practice team, peer health coaches and community support workers in an integrated manner.
The role has three aspects:
1) Working with patients providing brief psychological interventions particularly Focused Acceptance and Commitment Therapy (FACT) as well as delivering evidence based-group programmes.
2) Supporting, upskilling and educating the general practice team
3) Supporting local population health by developing group pathways based on clinic needs
Provision of training and supervision can also be part of the role.

PROGRAMME MANAGER - Mental Health Services
Alliance Health Plus is keen to appoint a Mental Health Programme Manager to work with our practices to support the delivery of Mental Health Services in Primary Care.
The successful applicant for this full time position will be an experienced mental health professional with extensive background in Mental Health. This role is a combined clinical and management involving clinical coordination, contract management, Mental Health service development and the direct provision of clinical consultations in several Alliance Health Plus practices.

We are looking for a Registered Nurse who is fluent in a Pacific language, with a passion for improving outcomes for pregnant Pacific women and their babies, to establish and lead an exciting new programme in our general practice network.
The programme’s objectives are to embed and support systems in our practices to help Pacific women be healthy during their pregnancies and prevent or manage their diabetes mellitus postpartum. The programme will be community and strengths based, underpinned by Pacific values and models of care. You will be well supported by the friendly AH+ team, a Governance Group and Community Support Worker to do the following:
Planning the programme’s implementation
Developing principles that underpin models of care based on Pacific worldviews of health and wellbeing
Working in close partnership with clinical colleagues inAH+ clinicsto embed the new models of care and supporting practices to maintain quality systems
Working in partnership with women and their families to achieve pre-pregnancy health and well-being, and postpartum screening and management of diabetes mellitus.
Working with a Community Support Worker to help women access key community and secondary services
Working with a co-design team to develop certain key parts of the programme
Upskilling practice staff when needed
