The days were long and Joe’s team worked from 8am through to 7pm daily in order to help as many people as they could, resulting in the highest number of registered measles immunisations in the surrounding area, during that time period. Team 13 triaged and assessed patients as they presented and collectively administered over 1,000 measles vaccines throughout the week.</div><div>Given Joe’s experience as a nurse in the primary care and community setting in NZ, he was able to support the team by providing guidance on Cold Chain procedures and delivering vaccination education including potential reactions including anaphylaxis, and the appropriate response.</div><div>With his depth of knowledge on Cold Chain procedures, Joe was asked to spend a day on the island of Savai’i to provide education and refresher training to the local nurses. This was to ensure the storage and transportation requirements aligned with best practice protocols which is critical in preserving the efficacy of the MMR vaccines.</div><div>Joe acknowledged the skills, co-ordination, dedication and hard work of the nursing and medical teams in Samoa, as well as the team-work that is evident both at the front-line and behind the scenes.</div><div>The situation in Samoa is heart-breaking and we send our love and prayers to everyone living in Samoa as well as those who are working hard to help protect the people of our dear Pacific neighbour.</div><div>The death toll currently stands at 70 with over 4,500 cases reported since the outbreak began.</div></div>]]></content:encoded></item><item><title>Wellness Support improves Mental Health care for our Counties patients</title><description><![CDATA[In 2018 Counties Manukau Health introduced the Wellness Support programme; a new approach to providing primary mental health care in general practices which is based on the principles of kind, clear, continuous and effective care for patients.We are pleased to announce that 100% of our Alliance Health+ CMDHB practices are now up and running with the Wellness Support programme! A huge THANK YOU to everyone involved with the transition.</div><div>The programme offers improved access for patients with mild to moderate presentations and those under the age of 18 years, with free mental health support including extended 30 minute GP or Nurse consults. Prior to the introduction of Wellness Support, patients only had access to funded mental health care in general practices if they were diagnosed with depression and were over 18 years of age.</div><div>In its first year, Wellness Support has reached in excess of 4,300 enrolled people and provided more than 10,000 consultations. The programme supports a team approach, and nurse consults have more than tripled from the previous CCM Depression programme. As a Pacific-led PHO with a mission to transform the health and wellbeing of Pacific and high-needs communities, the partnership is one that makes sense. In this partnership, we have a real opportunity to help significantly reduce health inequities for our Pacific population, while supporting our Practice network and enrolled patients, with the delivery of an exclusive health and well-being service, a variation of which, we can already see working at a grass-roots community level.</div><img src="http://static.wixstatic.com/media/4dbccf_2fc195559c7244ca857fdee73ffce4a3~mv2.jpg"/><iframe src="https://player.vimeo.com/video/372977814"/><div>Founder of BBM, Samoan boxer Dave Letele, is the first to tell you that four years ago he was in a bad way – 210kgs, broke, broken and broken-hearted having lost his business, family and his home. He turned his life around and now spends it, helping others do the same. Driven through the Facebook group @BBM Motivation Dave has created an online community with over 13,000 members and his boot camps attract those who want to get healthy in a safe and supportive space.</div><div>BBM runs at least 17 FREE boot camps weekly, in three locations in South, West and Central Auckland and participants are welcomed into the BBM family that lives by the culture of #noexcuses (and #nojudgements). Supporting the boot camps or for those who don’t live close by, BBM also has an app based program which includes fitness programs, healthy meal plans and more. Watch this space for more developments on our work together!</div><img src="http://static.wixstatic.com/media/4dbccf_451204107c5e4b8a86d8d0404be28903~mv2.jpg"/><div>An insight into a BBM boot camp session:</div><div>A couple of (brave!) staff members from AH+ attended a Monday evening boot camp session in Manukau at ‘BBM HQ’, although we left with sore bodies we also left with a full hearts – the boot camp space was filled to capacity with all shapes, sizes, ages (including children – at BBM sessions children are welcomed, in fact bringing your kids is encouraged so they can see their parents exercising and looking after their health – leading by example). </div><div>We found ourselves a space over in the corner, as far towards the back as was possible – nervous and a little shy! As it turns out there was no need for this, at BBM everyone is made to feel like part of the family, and it became clear as the class started, just why their attendee numbers are so high and why people keep going back. AH+ staff are a diverse fusion of cultures from Tonga, India, Niue, Cook Islands, China, Fiji, Samoa, Scotland, the Channel Islands, America and Aotearoa woven together to create the Alliance Health Plus team. This prescribing capability supports the New Zealand Health Strategy - enabling registered nurses to fully use their skills and training to provide the right care at the earliest opportunity. The Council has focused nurse prescribing to complement the roles of other health professionals and target areas where people have challenges accessing healthcare and medicines.</div><img src="http://static.wixstatic.com/media/4dbccf_77123b772ac342d0a5ea124821cc1a60~mv2_d_1380_1840_s_2.jpg"/><div>Above: AH+ is proud to acknowledge network nurse, Sushma Sharma from Bishop Medical Centre who was one of the 56 Community Nurse Prescriber graduates from the trail run in 2017.</div><div>The Council, in partnership with Counties Manukau Health and Family Planning New Zealand, completed a trial and evaluation of a workplace-based education programme to prepare registered nurses prescribing in community settings such as primary health care, schools, public health and sexual health. </div><div>Nurses completed a work-based education programme and supervision before applying to prescribe from a small list of common medicines rather than use standing orders or ask a doctor to sign the prescription. Fifty-six nurses participated in the trial. These nurses were from Family Planning clinics around the country, as well as practice nurses, school nurses and public health nurses in Counties Manukau. All nurses were authorised to prescribe by November 2017. </div><div>Their prescribing practice was evaluated for a minimum of three months and the evaluation concluded that:</div><div>registered nurses prescribed safely using treatment guidelines.nurse prescribing increased patient accessibility to medicines, decreased patients’ out-of-pocket expenses, and freed up time for doctors to see more patients.nurses had high professional and job satisfaction and felt more valuable to the system.education prepared nurses effectively for prescribing, and after the trial almost all nurses remained highly confident in their prescription abilities.most nurses felt well supported in clinics when they needed reassurance or help.</div><div> The assessment process demonstrated that nurses prescribed antibiotics in adherence to antimicrobial stewardship principles and ensured patients were appropriately prescribed medicines according to their needs.</div><img src="http://static.wixstatic.com/media/4dbccf_4245b898819b46aeb72cc2de632ca515~mv2_d_1980_1485_s_2.jpg"/><div>Above: Some of the RN graduates of the trial with Hon Minister Jenny Salesa</div></div>]]></content:encoded></item><item><title>To'o Vaega Graduates NZIS Personal Trainer Level 4</title><description><![CDATA[We are proud to celebrate yet another achievement of one of our AH+ staff. Jo is a member of the Pacific People’s Health Advisory Group (PPHAG), created through the facilitation and support of AH+ and suffered rheumatic fever as a child. As a result of this he has had ongoing heart problems. Weekly motivational tips and advice was displayed in the staff kitchen and weigh-ins happened every Friday, followed by the anxious wait for the results showing overall % weight-loss per participant.</div><div>Congratulations to everyone who took place and contributed to the 40% combined body weight that was lost over the 8 weeks!</div><div>Although the challenge has finished, monthly weigh-ins will continue as motivation for those who wish to continue. Since then, South Seas has developed and delivered a number of healthcare services, their General Practitioner clinic currently provides comprehensive primary care services to over 5,500 patients, with an outreach and home visiting nurse service. </div><div>Past innovations by South Seas include setting up the first Community Lactation Clinic with Turuki Health care and training the first Māori and Samoan Lactation consultants in New Zealand. In addition to being a Whānau Ora service provider, South Seas social workers and community workers provide Healthy Housing intervention (AWHI) and their health campaigns for rheumatic fever are available in seven schools in South Auckland through the Mana Kidz programme.</div><div>On Saturday 9th February 2019, a 20th birthday celebration marking this impressive milestone, took place at Te Puke o Otara Community Centre. More than 300 people were in attendance, including the Associate Minister of Health, Honourable Jenny Salesa as the keynote speaker and the Minister for Pacific Peoples, Honourable Aupito William Sio, who had the honour of cutting the cake. </div><div>CEO of Alliance Health Plus PHO Wayne Williams, congratulated South Seas for their accomplishments to date with the gift of a painting. With approximately 200 guests in attendance the evening was a chance to mingle, unwind, laugh and collectively celebrate the success of the network and the individual award-winning practices.</div><div>Attendees which included VIP guests such as, Honourable Minister Jenny Salesa, Honourable Aupito William Sio, Matafanua Hilda Fa'asalele, Fepulia'i Margie Apa and guest speaker Pat Snedden, as well as Ministers and their wives from the Tongan and Samoan HVAZ Churches, representatives from regional DHBs and of course members of our practice network and providers. This was to encourage nurses to take the lead, lead by example and fully engaged in participating in the design and development of health care services. Areas of focus included health workforce development, transforming leadership, enhancing clinical care, extending professional boundaries, health response to climate change, disasters and outbreaks. The forum provides a regional platform for nurses to share knowledge, innovation and experiences from the diverse cultures and territories across the South Pacific.</div><div>There were attendees and presenters from across the Pacific nations, New Zealand, Australia and also the World Health Organisation, International Council of Nurses and the International Council of Midwives. We were privileged to have Elizabeth Iro, from the Cook Islands, as the keynote speaker, who is the first Pacific Chief Nursing Officer for the World Health Organisation. What a great honour to have one of our own Pacific nurses leading an International organization and being the voice for nurses across the world.</div><div>In attendance from Alliance Health Plus Trust was the Pacific Integration and Development Services Manager, Metua Bates, Loli Mesepa Channing (Parish Community Nurse) and Pauline Fuimaono Sanders (Nurse Leader). Loli and Pauline presented at the Forum and contributed to the rich discussions about Pacific nurses positively contributing to Pacific health outcomes. Loli presented on her experience as a Parish Community Nurse in New Zealand. This unique role works with Pacific church communities to deliver self-management health education and support members to achieve positive health outcomes. The Parish Community Nurse model could be successfully applied within a Pacific context as a response to addressing NCDs. </div><img src="http://static.wixstatic.com/media/4dbccf_296286f364a94b7d9a0372ab840561a8~mv2.jpg"/><div>From left: Pauline Fuimaono Sanders, Metua Bates, Loli Mesepa Channing</div><div>Pauline presented on her involvement in the New Zealand RN Designated Community Nurse Prescribing trial in 2017. This trial focused on nurses working in General Practice, Schools, Public Health and Family Planning settings in the Counties Manukau DHB area. Five nurses from the Alliance Health Plus Trust network are graduates and are successfully utilizing their new skills. Pauline invited Piri Puata, one of the nurse prescribing graduates, to share her experience about how this has positively influenced her nursing practice. Key benefits are autonomous nursing care, timeliness and affordability of care, reaching communities of need and high satisfaction from patients and their families. This level of prescribing could be successfully applied to Pacific regions and improve access to healthcare for communities.</div><div>The forum is an excellent opportunity to share ideas, resources, knowledge and experience. These new insights and relationships can support enhancements in practice, improved service delivery, changes in policy to ultimately lead to positive patient experience and health outcomes. Coined ‘Take to Heart’, the seven month campaign which runs from May till November, promotes awareness of rheumatic fever as well as its causes and how it can be prevented.</div><div>Engaging with 18 churches across the Auckland region, spanning three DHBs and encompassing Samoan, Tongan, Cook Island, Niuean, Fijian and Tuvaluan parishes, ‘Take it to Heart’ empowers each church to strongly engage their people to interpret the campaign’s five key messages. The Churches have embraced this opportunity, and are delivering the key messages to their communities through an innovative mixture of song, dance, theatre and poetry – a formula with which the PIDS team has already had great success.</div><div>To date, ten Churches have delivered events across Metro Auckland with between 1,500 to 2,000 people in attendance. These events are highly anticipated by the participating churches and their communities, with the hard work invested by the churches, evident in the energetic and passionate performances. Surveys that are completed at each event show a heartening increase in awareness from both the participants and the audience, as well as an increasing understanding around Rheumatic Fever causes and prevention.</div><div>Following the recent event held at EFKS Papakura Church, Faimafili Tupu from Auckland District Health Board shared her appreciation for the campaign -</div><div>“Thank you for the opportunity – to witness and to strengthen what we continue to voice – that if we empower the community with the messages for any project/programme – they will stand up, take control and deliver it in ways that will reach the audiences present”. - Faimafili Tupu, ADHB</div><div>Community events are being supported and campaign messaging strengthened, through interview segments on radio 531pi. Regular spots feature health professionals from AH+ pacific providers discussing key messages which are broadcast to radio 531pi’s many Pacific listeners. Many of these conditions are often grouped within the same person. Historically visits to GP practices have been dominated by face-to-face consultations with GPs with a 15 minute duration. There is now a compelling case to change this Model of Care (MOC) to one that is more appropriate for the Pacific patient and their family; get better results; and utilise the skill sets of many health practitioners, not just GPs. The trust provides a home visiting programme that focuses on improving children's growth and health, learning and relationships, family circumstances, environment and safety. It helps families/whanau who are struggling with challenges that make it harder to care for their baby or young child. Our values as a PHO mesh well with Family Start, so we decided this year we would donate instead of running an in-office secret Santa” says AH+ CEO Wayne Williams.</div><div>“I just can’t believe the generosity of the team at AH+. We totally did not expect this! Our families out there, are really struggling as Christmas nears. These donations will go a long way in making a big difference” says Colleen Fakalogotoa.</div><div>The donation drive was such a hit, that staff in adjacent offices where AH+ is located offered to donate as well. “It was awesome to see what the team downstairs at AH+ were doing and we felt that we would chip in. We usually do a money run around the office to donate, but I think we might start doing the same as Alliance Health Plus” says Shelly Witehira from Watts and Hughes.</div><div>The Family Start team broke into song as a thank you to the AH+ staff for having donated canned food, children’s toys, cleaning products, clothing, homeware and other long-life food products.</div><div>Family Start are open to donations and are located at 11 Dalgety Drive, Wiri. We had more awards this year recognising a range of clinical and community achievements and more representatives from across the health sector.</div><div>Entertainment was provided by St Giles- Mt. Albert Church Choir, who came runner ups in the Rheumatic Fever Sing Off hosted by AH+ with their original song 'Call the Doctor'. We also had the Macleans College Lion Dancers with their colorful display of inviting in luck and prosperity with an original dance.</div><div>The highlight of the night was the sobering story of struggle, resilience and determination within our primary health system delivered by Naoupu Tupuivao. Naoupu talked of her struggle trying to maintain the health of her children in unsuitable rental properties, a lack of understanding of the health system and a cultural struggle with mental health. She was able to take control of her and her children's health, as well as their lives with the help of South Seas Healthcare, one of the practices within the AH+ Practice Network.</div><div>We also recognised an important and amazing achievement of one of our clinical nurses within our Practice Network. The New Zealand Nurses Organisation awarded their presigtious Young Nurse of the year award to Jess Tiplady of Greenstone Family Clinic, Manurewa. Jess has been a huge part, and led regional initiatives around child skin infection and eczema.</div><div>Feedback from the night has been nothing short of humbling and glowing. We believe we have found the formula for delivering successful communications to our people and getting them to understand and act upon the messaging around Rheumatic Fever” says Sinia Saafi, Team Leader of Pacific Integration Development Services (PIDS) team of AH+.</div><div>The strategy has seen over 12,000 reported individual participants involved in the innovation projects- not including thousands more who have heard through word-of-mouth about the key messages around Rheumatic Fever. The PIDS team encourage and work alongside schools, community groups and churches to provide funding on how to formulate, take ownership and lead events such as singing competitions, theatre or poetry recitals that suit their respective audiences on Rheumatic Fever education.</div><div><img src="http://static.wixstatic.com/media/4dbccf_944d56afb27e4cde83925b760cff5e10~mv2_d_5472_3648_s_4_2.jpg"/><img src="http://static.wixstatic.com/media/4dbccf_fe10e4123c7a48d69c0c26bf59f3deaf~mv2_d_5472_3648_s_4_2.jpg"/><img src="http://static.wixstatic.com/media/4dbccf_6486f56e09c24a4a88c51729f342f3e3~mv2_d_4032_3024_s_4_2.jpg"/></div><div>“Although we are all Pasifika, it is important to remember that we all have our own unique cultures and although the majority of us may be Christian we all have our o</div><div>wn specific denominations that we are a part of. Within this there is also the hierarchical element. So, it is vital that these aspects are recognised when encouraging our families and communities when health messages are being delivered” says Sinia.</div><div>The team have been targeting specific hotspot areas around the Auckland region where Rheumatic Fever rates are high, such as Kelston where one successful event attracted over 500 attendees from the local Samoan community and Papatoetoe where a Rheumatic Fever performance night saw 300 attendees gathering from the local Tongan community.</div><div>The projects have become such a hit with organisations that calls from as far afield as Taupo have been made requesting to be a part of the strategy. Requests such as this show that these projects are desperately needed elsewhere outside of Auckland in order to tackle the rates nationally.</div><div>The PIDS team of AH+ believe that with their past, current and future events they will be a big part of reducing the high numbers of Pasifika contracting the disease. Saveasoi Temukisa Amituanai-Andrews, the Rheumatic Fever Coordinator for PIDS says “I think we’ve found the key to getting the messages out there, understood and acted upon. AH+ is only 0.25% away from achieving this target and will be able to do this early.</div><div>Philippa Little, Project Manager for the Patient Portal implementation at AH+ says: “The competition was a bit of fun to get the practices more involved with the portal implementation.
